Bespoke Specs for Astigmatism
Mission for Vision uses a frame and lens system that enables the correction of more complex refractive errors including high degrees of astigmatism. The lenses are pre cut and are fitted to frames for round lenses. The lenses can be rotated to align with any axis required. These have proven to be hugely popular as we can now correct for most ametropias we are faced with in the remote areas we visit where no lens cutting facilities exist.

Fan and Block used to be a common method for correcting astigmatism but has been superseded by the cross cyl technique. It is however still a useful method especially when dealing with larger cyls and sometime patients finds the fan and block method less confusing. Its certainly a useful backup especially in situations where hi tech equipment is not available.

    It seems to me that in order to do the reversal test, a -0.25 cylinder at 100 deg (perpendicular to the original sharpest line) should be added, instead of a -0.25 sphere. Because the former would be an over-correction, causing the anterior focal line to go further back, becoming the new posterior focal line, thus causing a reversal, while the latter would just move the entire pencil closer to the retina, therefore shouldn't cause a reversal. Am I missing something?

    Hi thanks for this. Is there anyway, for bifocals where there is reading lens, which numbers to add that to in order to get the reading glasses prescription? For example, with a distance prescription of -0.50, +2.75, 017 (vs it's transposition +2.25, -2.75, 107) , which transposition do I add this 1.75 number to in order to get the reading lens spherical prescription or does it matter?

    Please don't ever take this video down. I link to it in my optical classes when we cover different types of edgers. We have a mobile health lab at our college that allows us to give eye exams and glasses to those in need (Project Sight at Durham Tech in Durham, NC), and this video serves to educate our students about the global need for people with their skills to help the greater community. It's a shame Ian's life was cut short, but his legacy still lives on. ❤

    principle: Find BVS with a subjective or ret test. bring 2nd FL to ret with pos. bring 1st FL to ret with neg. test: occlude one eye. use BVS fo estimate the cyl power, add half the pos cyl to get 2nd FL onto ret. adjust the axis. (at 90* to test chart axis). add 0.5 sph plus as a check for later. now onto blocks. need to add minus cyl until blocks are equally blurred. the check: remove the 0.5 sph and there should be a reversal of block clarity.

    Very useful video. In this example, the sph is a minus(-5.00), so every time you increase cylinder power by -0.50, you reduce sph power by 0.25. What about if the sph power is +5.00, when you increasing the cylinder by 0.25, will you decrease sph to +4.75 or increase it to +5.25? Confuses me alittle bit but I understand the technique

    Thank you Ian, I now do my own refraction, using my own test lenses. Starting at 20/60, or -0.75 diopters, I got a change of +1 diopter, to 20/20, at measured +0.25 diopters. In other words, I got myself out of myopia. Thank you for helping me achieve this scientific result!

    Great tutorial! Your example featured a + sphere example. In myopic minus patients, the same increase in cyl power (once best vision sphere has been determined) would follow the same rule for compensating sphere power with higher cyl power but in this case decrease the sphere by 1/2 of the cyl (by 0.75 as in your example)? Or does this only apply to plus powers? So for a -3.50 best sphere, every 0.50 diopter increase in the required cyl would drop the sphere by 0.25? If not the case, can you please explain why?
